Maybe you didn't install one of the attributes weight product info modules ? (content or layout )
Or check if you have the override product_loader uploaded or if you have another product_loader class in the override directory which loads after the product_weight override class. If yes, you need to combine the modifications of both into one override product_loader class

Its a "clean install" with only QtPro and your Attributes Weight.

both product_loader.php are similar and adding these two lines:

'weight' => ((($attribute['option_weight'] > 0)? $attribute['option_weight'] : (($attribute['products_options_values_weight'] > 0)? $attribute['products_options_values_weight'] : '0' )) ),
'weight_prefix' => ((!empty($attribute['option_weight_prefix'])? $attribute['option_weight_prefix'] : (!empty($attribute['products_options_values_weight_prefix'])? $attribute['products_options_values_weight_prefix'] : '+' )) ),

makes them equal and now getting:
Notice: Undefined index: option_weight in /xxx/includes/system/override/qtpro/product_loader.php on line 74
Notice: Undefined index: products_options_values_weight in /xxx/includes/system/override/qtpro/product_loader.php on line 74

I can't reproduce that error on my test store.
Make sure you have one of the options weight product info modules installed and the 2 columns added in the products_options_values and products_attributes tables.
Once installed and the columns added in the database, you can remove the weight module and it should work with the qtpro module.
Anyway these are only notice errors and it should work.
