It seems you missed something updating to 1.0.8.21.
"customer_data_groups_width" was introduced in Phoenix 1.0.8.21 and the error is due to a problem with your Phoneix core.
You should get the same error in the core create_account.php page. The relevant code is identical in both pages, checkout_guest.php and create_account.php
