I believe I have it installed correctly so I think core changes between 1.0.7 and 1.0.8.4 may have introduced this error. Is it simply placing the "/templates/slim_checkout/includes/template.php" in another location?

It’s not updated yet for 1.0.8.4, please see the Phoenix version label. It’ s labelled for 1.0.8.0.
It will be probably not updated until 1.0.9.0 release.
