Problem with State Import box not showing in Update Address Book Entry

What is happening:
with State set at false I get no inpiut box for state but do get an update button
with state set at true I still get no input box for state but also get no update button either
In both cases Country is set to United States

Which file is the input boxes coded in so I can add the state one
The State box is there for the create_account.php box and works as it should when United States is selected

Make sure the State module is set to show on "address_book" Page.
