Then your configuration table is not loaded. It has nothing to do with slim checkout or any other addon.
Check in your database in the hook table if you have this entry:
hooks_id: 6:

If it is missing, try to recover an older backup of your database which includes this entry.

If you do not have a backup you can use the following hook table backup with all core hooks for Phonix 1.0.8.6-1.0.8.9:

If you have the hook entry, independent of the id, it makes no sense to try this.

I'm using the correct db.

Also, I just installed 1.0.8.9 on my home CentOS 8 server. Getting the blank page also on the shop side. Admin side good.

Still trying to install a fresh 1.0.8.9 on Godaddy but not having any luck.

Forgot to mention, I was missing several hooks from when I compared the number. That's why I ran the .sql.

Still trying to install a fresh 1.0.8.9 on Godaddy. The automatic install doesn't work so I tried a manual install by uploading the files and running the phoenix.sql in the install directory.
