check error_log file and post the errors here if there is

Ultimate SEO is not updated yet for latest Phoenix. It’s flagged for Phoenix 1.0.8.4.
Check always if the Addon you download is flagged for your Phoenix version.

OK I need to correct myself. I have isolated the problem to the ultimate seo addon. It intermittently will not allow a product to be added to cart. Site works perfectly when add on is not enabled. Enable addon I can add 1 or 2 items to cart. Then if I try to add more they will not add to cart. It's kind of an intermittent problem. After I can no longer add to cart the only way to fix the problem is Unable the addon. When I do this the website works perfectly. Reenable and the same issues again.

I believe the current version for Phoenix is 1.0.8, perhaps as another person indicated the addon is not updated.

So does this mean each time the website software is updated the addons will break?

Look at the page and you will see the what version the addon is good for in orange shaded download buttons.
app.php/addons/free_addon/ultimate_seo_urls
The latest for this addon shows 1.0.8.4. That means it is good for 1.0.8.4.
It might work on later versions, it might not so beware of using addons not flagged for your PhoenixCart version or at least ask the author before using them.

Not necessarily so but always a possibility with non-core addons/modules so think carefully before loading up your store with them unless you are willing to pay a developer to fix them when free support for them is unavailable..
Addons published by Certified Developers are regularly updated so they can be used with more confidence.
As far as the addon in question is concerned, if you really think it benefits your store, there is a free alternative as already linked in a previous post by Omar_one (as well as advanced paid for versions) published by a Certified Developer.
