The name in the second column means it appears it is working.
Contact your host as @cbs suggests and see if they can spot anything untoward.

Contacted my host and they did the following
It appears the contact form doesn't actually include this sort of information by default for some reason.
There is a workaround that I have implemented for you and it does seem to be working.
The changes are made to a file called: notifications.php which is in this directory:
/home/aztecmodelsco/public_html/includes/system/versioned/1.0.8.3/
Then this section of code is changed from:

Code: Select all

// Instantiate a new mail object
      $message = new email();
      $message->add_message($email_text);
      $message->build_message();
To:

Code: Select all

// Instantiate a new mail object
      $message = new email();
      $email_text = "Customer Email: " . $from_email_address . "\n\n" .   $email_text;
      $email_text = "Customer Name: " . $from_email_name . "\n\n" . $email_text;
      $message->add_message($email_text);

That change looks like it adds the name and email to the text of the email that is sent to you.
Was that the problem all along ?

Yes it put the details in the message, where as before I was not even getting the details.

When I received the email the from address was my contact address and I had no name, so just the message body was coming through.
